Sending Laboratory Work:

If you are sending digital files, Follow the steps below.

STEP 1:

• Access the Horizon Pro Laboratories Portal and click on the link 'New Order’

STEP 2:

• Fill out the order form and upload ZIP File or STL files of intraoral scans

Now you can place an order for a pick up with us for physical impressions. Just follow these 2 simple steps:

STEP 1: Click the link below to download and complete our Horizon Pro Laboratories Prescription Form. Then, attach the form to your patient's case.

STEP 2: Contact us for pickup:

• Access the Horizon Pro Laboratories Portal and click on the link 'pick ups'.

• Please write our Laboratory address on the parcel that you will be shipping the case in so that it will be ready for pick up.
